How to Submit Your College Report
Please allow 5 business days for application materials to be added to your checklist.
A dean, registrar, advisor, or another college official who has access to your records must complete this report. The form should be filled out as completely as possible by the official; the sections regarding academic standing and disciplinary record are required. We cannot review your application without a completed College/Registrar Report. Please use the Common Application College Report form.
The Registrar/College Report can be printed from your online application account, completed, and must be submitted to us by a college official. They can submit that to us by mail, or by email to applicant@cornell.edu.
