How to Submit Your Mid-Term Report

Please complete the Mid-Term Report by listing the ongoing courses you're enrolled in for the spring term. The grades entered can be official midterm grades or estimates of your performance based on assignments you've received grades for thus far.

Please note that the Mid-Term Report is only available after you’ve submitted your application and logged in to your Applicant Portal.

Do not submit this form until you have an official or estimated grade in each class you're taking for the spring. If you aren't enrolled in spring courses, you can note that in this form. If you are unable to provide a grade or assessment for a class, you must provide a written explanation, which can be submitted through the document uploader in your Applicant Portal. 

Final grades will later be reviewed to ensure consistency with the information provided in the Mid-Term Report. 

Quarter/Trimester System Applicants: You can submit mid-term or final grades for your second term courses. Please list your third term courses in the “Upcoming Coursework” section of the form. 

Summer Courses: Include only summer courses you plan to complete, and list them in the "Upcoming Coursework" section of the form. Please note that planned summer courses are reviewed on a case-by-case basis and may not meet transfer requirements.
